Which intervention is recommended after ROSC to maintain a systolic blood pressure of at least 90 mmHg?

Administer 1-2 L fluid bolus to maintain SBP ≥90 mmHg

After ROSC, the priority is to ensure adequate perfusion of organs, including the brain and heart. If the patient is volume-responsive, giving a fluid bolus to improve preload can raise cardiac output and lift the systolic blood pressure toward 90 mmHg. A 1–2 L fluid bolus is a common starting maneuver to achieve that target without immediately resorting to vasopressors. While ensuring oxygenation is important, simply increasing oxygen to 100% doesn’t directly raise blood pressure. If hypotension persists after fluids, vasopressor support would be considered, but the first step to reach the SBP goal is fluid administration. Withholding fluids and observing would leave perfusion compromised, making it an inappropriate first choice.
